Tuition - $2500 USD for each individual registrant.  

As course enrollment is limited to 16 people, early registration is recommended. If paying by check or money order, please make tuition check payable to, “Department of Radiation Oncology - Wake Forest University Health Sciences,” including the name of participant and last five digits of the participant's social security number.

Please note: All cancellations must be received two months prior to start of course or tuition will be applied to a future training course.  If at least six participants have not registered for a course date within 2 months prior to the course then we reserve the option to cancel that course date with full tuition re-imbursement.  Two months of lead time is needed to fairly allow participants to make appropriate travel and clinical coverage arrangements.

Course tuition will include course fees, instructional materials, and meals (breakfast/lunch Day 1 and 2) and dinner with faculty on evening of Day 1.

HOUSING and TRANSPORTATION

A block of rooms have been reserved at The Graylyn Conference Center: http://www.graylyn.com/

Graylyn has some of the following amenities and services: complimentary Wi-FI, complimentary morning coffee and tea, etc. at a rate of $189 plus 12.75% occupancy tax.  We will also arrange local transportation to and from the airport and to and from the Medical Center and do recommend all participants plan on staying at this historic hotel during the Wake Forest SBRT Course.  We will make your hotel reservation and provide you with a confirmation number as well as your local transportation information a week or two prior to your travel.  We will need you to provide us with your flight information so the appropriate arrangement can be made.  Please note participants are responsible for all transportation to Winston-Salem and hotel costs and should provide a major credit card for these services.  Unless indicated differently a standard single room will be provided

Wake Forest Baptist Medical Center Radiation Oncology  is an American College of Radiology (ACR) accredited radiation oncology center and is an NCI desginated Comprehensive Cancer Center.

 

 

CME Credits

CAMPEP - This program has applied to CAMPEP for MPCEC credits. Attendees completing the full course will receive 14.5 credits.
Physician CME is through the individual participants national medical association self reporting requirement systems.
